Certificate of Discharge (F. 1394) is forwarded herewith for retention.
You are responsible for keeping it in safe custody and in good condition. Duplicates cannot be issued. You are warned that it is an offence to make, or allow to be made, any alteration or erasure in, or otherwise tamper with your Certificate of Discharge, or any other official document in your possession.
The envelope in which this is forwarded should be used to Keep [sic] the document clean and intact.
To obviate any possible misuse of Form 1394, you should put your signature in the space provided marked “X”.
In any communication concerning yourself, please quote your official number and write your name clearly.
