Asks recipient to fill out form which would be in recipients interest. Mentions that when he came back to Cosford the question of his future would arise. It would be easier to decide if they knew his preferences. On return he would be interviewed by a welfare advisory officer and could see a specialist if not satisfied with his recommendations.
