I am directed to inform you that Family Allowance and an Allotment of Pay at weekly rates specified below will be issued to you, with effect from 30.10.41. in respect of the above-named Airman, and a book of orders by which you can obtain payment has been sent to-day to the Post Office at Stapleford, Nottingham.
Marriage and one Birth Certificate [deleted] (s) [/deleted] submitted in support of the claim to Family Allowance are returned herewith.
[underlined] The Order Book will be handed to you on presentation at the Post Office of the enclosed Form 1511 (B) duly completed in Parts 1, 2 and 3. [/underlined]
